Acrobat Pro 7.0.9 Comment & Markup is missing from my menu bar and
preferences. I do not think it was ever present on my install from CS2 suite on
10.3.9Send for Review is on the Menu Bar but missing the icon. I have opened and
closed all the Toolbars. Anyone have experience with this behavior? I expect a
reinstall is in my future...

Control-click an empty area of the toolbar - you can then select the
toolbar ..
