If you have Acrobat Pro, you can create the form using the form tools available
in the application then enable it so that Reader users can save it.

Heather;
I'm assuming you have Acrobat Pro Mac edition. LiveCycle is a Windows only
product. My suggestion would be to design the form appearance document (layout)
in a layout software such as Illustrator or InDesign. Convert it to a PDF.
