You can do this to with version 2009, if you set your project for item 
numbering on a per part basis and you assign additional item numbers to the 
sub components under Multiple Catalog entries.  The balloon insert will 
detect the additional item numbers and insert them.  See the attached screen 
shot from AcadE version 2009.
