Personalized Chocolate Bar Tutorial

Years ago I had a tutorial which allowed me to make a personalized chocolate 
candy bar with a bite out of it. I have searched high and low and can't find 
this tutorial on any of my computers. Does anyone have it?

When I try and Google it, I get loads of chocolate candy bar wrapper 
tutorials, but what I am interested in is only for a graphical image of a 
Hershey-like rectangle shaped candy bar.
