Edit Menu -> Preferences -> General Tab -> user name field

I believe that warning is issued when someone enters their user/name
password incorrectly and fails to login. It can also occur when an
"opener database" or script tries to login with invalid credentials.
Googling around the internet seems to confirm this recollection.

If there is going to be a formal list it will be with the FM Server 9
documentation pdfs. But I wouldn't be surprised if there isn't
anything.

You can supress the warnings by having FM Server only notify on
errors.

A curious one. I hadn't found the fails to login possibility; that 
should be testable
