Role based Workflows

I'm trying my first roles based workflows and I'm struggling.  I can't
get the default approval to work.  I get approval start - suspended. 
If the role is no approval, it works.

nrfRequestDN. I don't understand this object.  The documentation
describes it as the Role request object dn.  If I use the default
request form, I am required to fill this in using a text box.  Does
that mean I can name the request anything I want?

Who or what are you sending the approval to? This is the in the workflow
designer Approval activity, Addressee.

Follow up question here.  If I can name it anything I want, is there a
requirement that the name is unique, or can I reuse it in workflow
after workflow?

Are you initiating the workflow yourself or are you requesting the role 
assigment through the role UI and letting the role system initiate the 
workflow that it needs?

Have you assigned default approvers in the role configuration and or 
approvers for the role?
