Setup cannot locate the MOM database

I am building the MOM environment in HMC 4.0 and have gotten the database
installed on the SQL server.  I am now trying to install the MOM core components
on the MOM server but it cant find the MOM database.  I have installed MOM
several times in the past and can't seem to get past this issue.

I rebuilt a new MOM server from scratch, and still no luck.  The only deviation
from the HMC instructions is that I installed the MOM database on a specific
instance on the SQL server, not on the default instance.  I have done this in
the past with HMC environments without issue.  TCP/IP connections are enabled
for the SQL instance, and all SQL services are started.  Anyone else seen
this?
