Forms

Good evening.

I have a very simple form on my website that was working fine until a recent 
Publish.
The form is at : http://www.apipartners.com/html/info_request_form_2.html

I am publishing one page at a time with updated info.
The Action in 'Forms Button Properties' is:  /bin/formmail.pl

Please let me know if you require any additional info to help me solve this 
issue.
