I had similar problems though not identical.

I would do deployments of application ears to preexisting application servers in
WebLogic 9.2. However, as something has changed with bea_wls_async_response in
WebLogic 9.2 compared to WebLogic 9.1 the deployment of the application ears
would suceed but startup of the applications would, for lack of a better
understanding, fail.  This, although the applications would show up as
<Active> in the Administration Console. The applications were even
testable using the WLS test points located within the Administration console.

However, after I would shutdown and restart the Application Server to which
these applications  were deployed the whole application deployment would
disappear, the ear file where it was deployed into the stage directory and also
the exploded ear file that WebLogic runs after an application has been
