Some thoughts:

-- The key problem seems to be that "the call from createTopicConnection
does not return", which would be unusual.   Is the application creating a
lot of connections rather than re-using them?   Does the problem go away when
JMX notifications are disabled?

-- I'm unfamiliar with JMX internals, so I can't tell if the JMX debugging
reveals anything abnormal.
