Hi,
It's good to ask for suggestions before going for a hosting provider.
I have never tried it but check http://www.discountasp.net/
Ask people for reviews and then decide your hosting provider. Do not rush into
it :)

I would highly recommend http://www.re-invent.com
I've had very good luck with them and support your requirements. Good luck in
your search!

Discountasp.net is the perfect ASP.NET host and they offer all you are asking
for.
SQL Server 2000 and 2005 are addons.
Some hosts can beat DASP on the price but not on the service.
Your SQL Server addons are NOT on the same machines.
You can manage your DB from the control panel or from remote management.
Even better, DASP is now offering Windows Server 2008/IIS7
