Couldn't have been easier. I was trying to do it within WP. Wouldn't drag,

There are other ways but dragging is easier.
The other way is to right click it and Edit it. You then add the files
as a Program. This is the same way that you add Features, Macros and
such. Barry details the method on his web site.

As Kenneth says, there are other ways to do it, but dragging the file to
the toolbar is very easy. You can change the default icon on the new
button that appears: See http://wptoolbox.com/tips/EditIcon.html
Also, if you want to load a /new/ document based on a /custom template/
(in contrast to your current request to load a existing document from
disc), such as a letterhead page, see "Load a letterhead - or any other
document based on a custom template - with a mouseclick or keystroke" at
http://wptoolbox.com/tips/LoadLetr.html
